If your Georgia Tech assigned technology is not working as it should, you can request assistance with this form. Some common examples of IT support incidents:

  • Computer is not working
  • Application missing
  • VPN problems
  • Printing problems
  • Email access issues
  • Banner access
  • GTReports access
  • Something is physically broken
  • Unable to print
  • Need to borrow a computer or iPad

New Employee IT Onboarding

The following form should be used when a new employee is hired and needs to be setup with multiple IT Services. This form will create a request for each of the services listed since the time to fulfill the request varies for each service offered.

To prevent confusion and duplication, this form should *only* be filled out by the Hiring Manager or a designated Administrator in the department where the employee will work. It is very important to only submit this form *once* for each new hire. Please use the provided CC: field to keep everyone in the loop about this process.

Phone Support

If you are having problems with your existing Georgia Tech telephone service, you can request a repair with this form. Some common examples of repair requests:

  • No Dial Tone
  • Voice Mail Issues
  • Cannot hear the other party
  • The other party cannot hear me
  • The line is noisy
  • A feature is not working
  • Telephone set is physically broken
  • Other parties cannot call my number
  • I cannot make calls
  • Dropped calls
  • Phone does not ring
